What is a Carer?

A ‘Carer’ is a person who provides unpaid support for someone who couldn’t manage to live independently OR whose health or well being would deteriorate without help

Pangbourne Medical Practice is compiling a Carers Register. The purpose of which is to identify and support carers better.

We aim to ensure carers have annual health checks, are offered annual flu vaccinations and are screened for problems such as depression and anxiety.

We also aim to check that appropriate benefits and allowances have been offered and that emergency plans have been discussed for times of crisis.

Do you identify yourself as a Carer?

If so please let us know by…..

  • Completing a short form in the waiting room
  •  Telling your GP at your next visit
  • Or by calling the surgery and informing a receptionist    Tel: 9842234
