We send relevant information, such as appointment reminders and test results, to you via text and email, please speak to reception if you wish to opt out of this service.

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• evidence if your name has changed due to marriage or by deed poll
